ABSTRACT

Abstract Pain after liver resection can be difficult to manage. Epidural anesthesia (EA) is an effective technique in pain control in this surgery. However, postoperative coagulopathy and hypotension due to autonomic nervous system block in high-risk patients, may result that the EA is an inadequate analgesic technique in according to enhanced recovery after surgery (ERAS) recommendations for liver surgery. Regional block techniques have been recommended for liver surgery in ERAS guidelines. Erector spinae plane (ESP) block is a recent block described for thoracic and abdominal surgeries and provides both somatic and visceral analgesia. We describe a high-risk patient with cardiac dysfunction and Parkinson's disease who underwent laparoscopic right liver resection for hepatocellular carcinoma. Satisfactory intra and postoperative analgesia was achieved by a combined continuous ESP block, transversus abdominis plane (TAP), and oblique subcostal TAP blocks. Surgery and postoperative period was uneventful. No opioids were administered during hospitalization. A combined of thoracic and abdominal wall blocks can be an effective approach for intra and postoperative analgesia in high-risk patients undergoing laparoscopic liver resection. Further clinical research is recommended to establish the effectiveness of the ESP block as an analgesic technique in this surgery.


Resumen El dolor posterior a una resección hepática puede ser difícil de manejar. La anestesia epidural (AE) es una técnica efectiva para el control del dolor en esta cirugía. Sin embargo, la coagulopatía y la hipotensión postoperatorias debido al bloqueo del sistema nervioso autónomo en pacientes de alto riesgo, puede hacer que la AE sea una técnica analgésica inadecuada, de acuerdo con las recomendaciones de la recuperación mejorada después de cirugía (ERAS, por las iniciales en inglés de Enhanced Recovery After Surgery) para cirugía hepática. Se han recomendado las técnicas de bloqueo regional para cirugía hepática en las guías ERAS. El bloqueo del plano erector de la espina (BEE) (ESP, por las iniciales en inglés de erector spinae plan block) es una técnica reciente, para cirugías torácicas y abdominales, que brinda analgesia tanto somática como visceral. Se describe aquí un paciente de alto riesgo con disfunción cardiaca y enfermedad de Parkinson que se sometió a resección la paroscópica del lóbulo derecho del hígado por carcinoma hepatocelular. Se logró analgesia intra y postoperatoria eficaz mediante una combinación de bloqueo continuo ESP, y bloqueos del plano transverso abdominal (PTA) y del plano transverso abdominal subcostal oblicuo. La cirugía y el periodo postoperatorio transcurrieron sin novedad y no se administraron opioides durante la hospitalización. La combinación de bloqueos combinados torácicos y de la pared abdominal pueden ser un abordaje efectivo para la analgesia intra y postoperatoria en pacientes de alto riesgo que se someten a resección hepática laparoscópica. Se recomienda continuar con la investigación clínica a finde establecer la efectividad del bloqueo ESP como técnica anestésica para esta cirugía.

ABSTRACT BACKGROUND: Over the next 20 years, the number of patients on the waiting list for liver transplantation (LTx) is expected to increase by 23%, while pre-LTx costs should raise by 83%. OBJECTIVE: To evaluate direct medical costs of the pre-LTx period from the perspective of a tertiary care center. METHODS: The study included 104 adult patients wait-listed for deceased donor LTx between October 2012 and May 2016 whose treatment was fully provided at the study transplant center. Clinical and economic data were obtained from electronic medical records and from a hospital management software. Outcomes of interest and costs of patients on the waiting list were compared through the Kruskal-Wallis test. A generalized linear model with logit link function was used for multivariate analysis. P-values <0.05 were considered statistically significant. RESULTS: The costs of patients who underwent LTx ($8,879.83; 95% CI 6,735.24-11,707.27; P<0.001) or who died while waiting ($6,464.73; 95% CI 3,845.75-10,867.28; P=0.04) were higher than those of patients who were excluded from the list for any reason except death ($4,647.78; 95% CI 2,469.35-8,748.04; P=0.254) or those who remained on the waiting list at the end of follow-up. CONCLUSION: Although protocols of inclusion on the waiting list vary among transplant centers, similar approaches exist and common problems should be addressed. The results of this study may help centers with similar socioeconomic realities adjust their transplant policies.


RESUMO CONTEXTO: Nos próximos 20 anos, estima-se um aumento de 23% no número de pacientes em lista de espera para transplante de fígado (TxF) e de 83% nos custos no período pré-TxF. OBJETIVO: Avaliar os custos médicos diretos do período pré-TxF sob a perspectiva de um centro de atenção terciária. MÉTODOS: Foram incluídos no estudo 104 adultos em lista de espera para TxF, com doador falecido, entre outubro de 2012 e maio de 2016, tratados integralmente no centro transplantador do estudo. Dados clínicos e econômicos foram obtidos do prontuário eletrônico e do software de gestão hospitalar. Os desfechos de interesse e os custos dos pacientes em lista de espera foram comparados através do teste de Kruskal-Wallis. Um modelo linear generalizado com função de ligação logarítmica foi utilizado para a análise multivariável. Valores de P<0.05 foram considerados estatisticamente significativos. RESULTADOS: Os custos com pacientes submetidos a TxF (US$ 8.879,83; IC 95% 6.735,24-11.707,27; P<0,001) ou que morreram enquanto estavam em lista (US$ 6.464,73; IC 95% 3.845,75-10.867,28; P=0,04) foram maiores do que com pacientes excluídos da lista por qualquer motivo, exceto óbito (US$ 4.647,78; IC 95% 2.469,35-8.748,04; P=0,254) ou daqueles que permaneceram em lista de espera ao final do seguimento. CONCLUSÃO: Embora os protocolos de inclusão em lista de espera variem entre os centros transplantadores, existem condutas semelhantes e problemas comuns devem ser considerados. Os resultados deste estudo podem auxiliar os centros com realidades socioeconômicas semelhantes na adequação das suas políticas de transplante.

ABSTRACT

Background: Some studies have indicated alcohol abuse as one of the contributors to the development of cardiovascular disease, particularly coronary heart disease. However, this relationship is controversial. Objective: To investigate the relationship between post-acute coronary syndrome (ACS) alcohol abuse in the Acute Coronary Syndrome Registry Strategy (ERICO Study). Methods: 146 participants from the ERICO Study answered structured questionnaires and underwent laboratory evaluations at baseline, 30 days and 180 days after ACS. The Alcohol Use Disorders Identification Test (AUDIT) was applied to assess harmful alcohol consumption in the 12 months preceding ACS (30 day-interview) and six months after that. Results: The frequencies of alcohol abuse were 24.7% and 21.1% in the 12 months preceding ACS and six months after that, respectively. The most significant cardiovascular risk factors associated with high-risk for alcohol abuse 30 days after the acute event were: male sex (88.9%), current smoking (52.8%) and hypertension (58.3%). Six months after the acute event, the most significant results were replicated in our logistic regression, for the association between alcohol abuse among younger individuals [35-44 year-old multivariate OR: 38.30 (95% CI: 1.44-1012.56) and 45-54 year-old multivariate OR: 10.10 (95% CI: 1.06-96.46)] and for smokers [current smokers multivariate OR: 51.09 (95% CI: 3.49-748.01) and past smokers multivariate OR: 40.29 (95% CI: 2.37-685.93)]. Conclusion: Individuals younger than 54 years and smokers showed a significant relation with harmful alcohol consumption, regardless of the ACS subtype. .


Fundamento: Alguns estudos identificaram que o abuso de álcool contribui para o desenvolvimento de doença cardiovascular, em particular doença arterial coronariana. Tal relação, no entanto, é controversa. Objetivo: Investigar a relação entre abuso de álcool após síndrome coronariana aguda (SCA) na Estratégia de Registro de Insuficiência Coronariana (ERICO). Métodos: 146 participantes do ERICO responderam questionários estruturados e foram submetidos a avaliações laboratoriais nas condições basais e 30 dias e 180 dias após SCA. Aplicou-se o Questionário AUDIT (Alcohol Use Disorders Identification Test) para avaliar o consumo prejudicial de álcool nos 12 meses anteriores à SCA (entrevista 30 dias após a SCA) e seis meses após a SCA. Resultados: As frequências de abuso de álcool foram 24,7% e 21,1% nos 12 meses anteriores à SCA e seis meses após, respectivamente. Os fatores de risco cardiovascular mais significativos associados com alto risco de abuso de álcool 30 dias após o evento agudo foram: sexo masculino (88,9%), tabagismo atual (52,8%) e hipertensão (58,3%). Seis meses após o evento agudo, os resultados mais significativos foram replicados em regressão logística para a associação de abuso de álcool e indivíduos mais jovens [35-44 anos, RC multivariada: 38,30 (IC 95%: 1,44-1012,56); e 45-54 anos, RC multivariada: 10,10 (IC 95%: 1,06-96,46)] e fumantes [fumantes atuais, RC multivariada: 51,09 (IC 95%: 3,49-748,01); e ex-fumantes, RC multivariada: 40,29 (IC 95%: 2,37-685,93)]. Conclusão: Indivíduos com menos de 54 anos e fumantes apresentaram uma significativa relação com consumo prejudicial de álcool, a despeito do subtipo de SCA. .

UNLABELLED: After the introduction of high active antiretroviral therapy (HAART), the human immunodeficiency virus (HIV) was no longer considered a contraindication for transplantation. Yet, liver disease in this population is characterized by an accelerated course that may impact on the waiting list. OBJECTIVE: To evaluate the experience in Argentina with HIV positive patients listed for liver transplantation. PATIENTS AND METHODS: We analyzed 52 HIV positive patients listed between July 2005 and March 2010 (Group HIV positive). Results were compared with 462 HIV negative patients included during the same period (Group HIV negative). Data were obtained from INCUCAI, the Argentinian procurement organism and from the Transplantation Centers. RESULTS: The etiology of liver disease in the Group HIV positive was hepatitis C 40, HBV 3, fulminant hepatitis 3, alcohol 2, retrasplant 2 and others 2. The mean MELD at the time of listing was 1615 (lower than 19 in 40 cases, higher than 19 in 8, emergency in 3) in the group HIV positive and 16.64 in the group HIV negative (NS). The outcome in the waiting list for HIV positive and negative patients respectively was: death 14 (27


) (NS), mean time from listing to death 270.70 298.11 days vs 267.29 266.53 days (NS), mean time from listing to transplant 70.26 74.05 vs 261 187.6 days (P < 0.01), mean MELD at the time of death 12.54 (13 cases lower than 15, 1 higher than 19) vs 19.6 9.7 (P < 0.05), mean MELD at the time of transplantation 24.33 vs 24.1 7.6 (NS). CONCLUSION: HIV positive patients have high mortality in the waiting list and low access to liver transplantation. MELD score underscores the severity of liver disease in this population when compared to HIV negative patients.

ABSTRACT

O transplante hepático inter vivos adulto expandiu as possibilidades terapêuticas para pacientes com insuficiência hepática terminal. A necessidade de um volume hepático adequado que será retirado do doador e necessário ao receptor limita sua utilização em alguns casos. Apresentamos um caso em que se utilizou dois lobos esquerdos de dois doadores vivos no intuito de prover parênquima hepático suficiente ao receptor.


Living donor liver transplantation expanded the therapeutic possibilities for liver failure patients. The necessary correct match between the liver mass donated and received sometimes limits its use. A case that was used two left liver grafts from adult living donors is reported.

CONTEXT: Living donor liver transplantation has become an alternative to reduce the lack of organ donation. OBJECTIVE: To identify factors predictive of early graft loss in the first 3 months after living donor liver transplantation. METHODS: Seventy-eight adults submitted to living donor liver transplantation were divided into group I with 62 (79.5%) patients with graft survival longer than 3 months, and group II with 16 (20.5%) patients who died and/or showed graft failure within 3 months after liver transplantation. The variables analyzed were gender, age, etiology of liver disease, Child-Pugh classification, model of end-stage liver disease (MELD score), pretransplantation serum sodium level, and graft weight-to-recipient body weight (GRBW) ratio. The GRBW ratio was categorized into < 0.8 and MELD score into >18. The chi-square test, Student t-test and uni- and multivariate analysis were used for the evaluation of risk factors for early graft loss. RESULTS: MELD score <18 (P<0.001) and serum sodium level > 135 mEq/L (P = 0.03) were higher in group II than in group I. In the multivariate analysis MELD scores > 18 (P<0.001) and GRBW ratios < 0.8 (P<0.04) were significant. CONCLUSIONS: MELD scores >18 and GRBW < 0.8 ratios are associated with higher probability of graft failure after living donor liver transplantation.


CONTEXTO: O transplante hepático intervivos constitui alternativa para amenizar a falta de doação de órgãos. OBJETIVO: Identificar os fatores preditivos da perda precoce do enxerto hepático nos 3 primeiros meses após transplante hepático intervivo. MÉTODOS: Setenta e oito adultos submetidos ao transplante de fígado intervivos foram divididos em grupo I com 62 (79,5%) doentes com sobrevivência do enxerto superior a 3 meses, e grupo II com 16 (20,5%) que faleceram e/ou apresentaram falha do enxerto até 3 meses após o transplante hepático. As variáveis analisadas foram: sexo, idade, origem da doença hepática, classificação de Child-Pugh, critério MELD, nível sérico de sódio pré-transplante e relação GRBW. O critério MELD foi categorizado em > 18 e a relação GRBW em < 0,8. Na avaliação dos fatores de risco para perda precoce do enxerto hepático foi utilizada a análise uni e multivariada. RESULTADOS: Critério MELD <18 (P = 0,001) e nível sérico de sódio >135 mEq/L (P = 0,03) foram maiores nos doentes do grupo II. A probabilidade de perda do enxerto no transplante hepático intervivos teve como variáveis independentes o índice MELD > 18 e a relação GRBW< 0,8. CONCLUSÃO: Os valores de MELD >18 e GRBW <0,8 estão associados com maior probabilidade de insucesso no transplante hepático intervivos.

Los avances en técnica quirúrgica e inmunosupresión han permitido sobrevida global de alrededor de 80–75% a 5 y 10 años, con calidad de vida cada vez mejor. Clínica Las Condes ha sido uno de los centros pioneros y líderes en esta materia en Chile. Summary: liver transplantation is already recognized as the best therapeutic option for end stage liver disease in our country and worldwide. The advances in surgical and immunosuppressive issues have allowed an overall survival of 80–75% at 5 and 10 years with improving patients' quality of life. Clinica Las Condes has been one of the pioneer and leading centers in liver transplantation in Chile. In 16 years of experience the liver transplant team of Clínica Las Condes-Hospital Salvador y Hsopital Luis Calvo Mackenna has performed 400 liver transplants, 212 of them in Clínica Las Condes.Objectivethe aim of this descriptive study is to present the results of liver transplants performed in Clinica Las Condes from December 1993 to December 2009.Methodsclinical data from 212 liver transplants performed in Clinica Las Condes during the observation period mentioned above were collected and analyzed using descriptive statistics, in terms of diagnosis, surgical techniques, and main post transplant complications. ABSTRACT

A falência hepática fulminante (FHF) é uma síndrome clínica com alta morbidade e mortalidade. A disfunção de múltiplos órgãos apresentada pelos pacientes com FHF é um dos mais intrigantes problemas do manejo em medicina clínica e requer cooperação entre intensivistas, neurologistas, hepatologistas e cirurgiões de transplante. A única intervenção terapêutica que provê benefício definitivo nos pacientes com falência hepática aguda avançada é o transplante de fígado de emergência.


The fulminant hepatic failure (FHF) is a clinical syndrome associated with high morbidity and mortality. The multiorgan dysfunction presented by patient with FHF is one of the most challenging management problems in clinical medicine and require very close cooperation in clinical medicine, the meurologist, and the hepatologist and transplant surgeon's. The only therapeutic intervention of proven benefit for patients with advanced acute liver failure is the emergency liver transplantation. We performed a retrospective analysis of patients with FHF and submited for liver transplantation at the Oswaldo Cruz University Hospital, Pernambuco University.

CONTEXT AND OBJECTIVE: Expanded donor criteria (marginal) grafts are an important solution for organ shortage. Nevertheless, they raise an ethical dilemma because they may increase the risk of transplant failure. This study compares the outcomes from marginal and non-marginal graft transplantation in 103 cases of liver transplantation due to chronic hepatic failure. DESIGN AND SETTING: One hundred and three consecutive liver transplantations to treat chronic liver disease performed in the Liver Transplantation Service of Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo between January 2001 and March 2006 were retrospectively analyzed. METHODS: We estimated graft quality according to a validated scoring system. We assessed the pre-transplantation liver disease category using the Model for End-Stage Liver Disease (MELD), as low MELD (< 20) or high MELD (> 20). The parameters for marginal and non-marginal graft comparison were the one-week, one-month and one-year recipient survival rates, serum liver enzyme peak, post-transplantation hospital stay and incidence of surgical complications and retransplantation. The significance level was 0.05. RESULTS: There were no differences between the groups regarding post-transplantation hospital stay, serum liver enzyme levels and surgical complications. In contrast, marginal grafts decreased overall recipient survival one month after transplantation. Furthermore, low-MELD recipients of non-marginal grafts showed better one-week and one-month survival than did high-MELD recipients of marginal livers. After the first month, patient survival was comparable in all groups up to one year. CONCLUSION: The use of marginal graft increases early mortality in liver transplantation, particularly among high-MELD recipients.


CONTEXTO E OBJETIVO: A utilização de doadores com critérios expandidos (enxertos marginais) é uma importante solução para a carência de órgãos para transplante. No entanto, isto suscita importante dilema ético porque esses órgãos podem aumentar a chance de insucesso do transplante. Este estudo compara os resultados do transplante de órgãos marginais e não-marginais em 103 pacientes portadores de doença hepática crônica. TIPO DE ESTUDO E LOCAL: Cento e três transplantes consecutivos de fígado por doença hepática crônica realizados no Serviço de Transplante de Fígado do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo foram retrospectivamente analisados, compreendendo o período de janeiro de 2001 a março de 2006. MÉTODOS: A qualidade do enxerto foi calculada utilizando sistema de pontuação validado. A doença hepática do receptor também foi classificada em MELD (Model for End-Stage Liver Disease) baixo (< 20) e MELD alto (> 20). Os parâmetros avaliados na comparação entre receptores de órgãos marginais e não-marginais foram: sobrevida do paciente em uma semana, um mês e um ano, pico sérico das enzimas hepatocelulares, dias de internação pós-transplante e incidência de complicações cirúrgicas e retransplantes. O índice de significância foi de 0.05. RESULTADOS: Não houve diferença entre os grupos quanto ao tempo de internação pós-transplante, pico sérico das enzimas hepatocelulares e incidência de complicações cirúrgicas. Em contraste, a utilização de enxertos marginais diminuiu a sobrevida global de um mês. Os receptores de enxertos não-marginais com baixo MELD apresentaram melhor sobrevida em uma semana e um mês do que os receptores de enxertos marginais com alto MELD. Após o período de um mês, a taxa de sobrevida foi similar até um ano em todos os grupos. CONCLUSÃO: Em conclusão, o uso de enxerto marginal aumenta mortalidade precoce no transplante de fígado, principalmente nos receptores com alto MELD.

Bilateral and symmetric globus-pallidus hyperintensities are observed on T1-weighted MRI in most of the patients with chronic liver failure, due to manganese accumulation. We report a 53-year-old man, with rapid onset parkinsonism-dementia complex associated with accumulation of manganese in the brain, secondary to liver failure. A brain MRI was performed and a high signal on T1-weighted images was seen on globus-pallidus, as well as on T2-weighted images on the hemispheric white-matter. He was referred to a liver-transplantation. The patient passed away on the seventh postoperative day. Our findings support the concept of the toxic effects of manganese on the globus-pallidus. The treatment of this form of parkinsonism is controversial and liver-transplantation should not be considered as first line treatment but as an alternative one.


Hiperintesidades simétricas e bilaterais dos gânglios da base são observadas em imagens de ressonância magnética encefálica (RM) ponderadas em T1 na maioria dos pacientes com insuficiência hepática crônica devidas ao acúmulo de manganês. Nós relatamos o caso de um homem, com 53 anos de idade, com um complexo parkinsonismo-demência rapidamente progressivo associado com o acúmulo de manganês no cérebro, secundariamente a insuficiência hepática. Uma RM encefálica foi realizada e foram observadas imagens hiperintensas/hipersinal nas imagens ponderadas em T1 no globo pálido e, também, na substância branca dos hemisférios cerebrais ponderadas em T2. Devido à falta de resposta ao tratamento clinico optamos pelo transplante hepático. O paciente faleceu no 7° dia de PO. Nossos achados corroboram o conceito dos efeitos tóxicos do manganês nos gânglios da base/globo pálido. O tratamento desta forma de parkinsonismo é controverso e o transplante hepático não deverá ser considerada uma opção terapêutica de primeira linha, porém como um tratamento alternativo considerando-se os riscos-benefícios dessa escolha.

ABSTRACT

The world over, liver transplantation has emerged a panacea for thousands of patients suffering from end-stage liver disease. The strides made in living donor liver transplantation (LDLT) by Asian centres particularly in Japan, Korea, Hong Kong and Taiwan made many Indian centres realise that in order to sustain liver transplant activity in the country, a similar solution had to be found. Even though LDLT is very resource intensive and requires skilled multidisciplinary manpower, 22 centres in India have performed liver transplants, of which 14 have performed at least one LDLT procedure. 140 LDLT procedures have been performed at our centre, of which 13 have been done in emergency circumstances. LDLT has certain advantages over DDLT. It allows for adequate preparation of the patient for elective transplant and recipients are not in competition with others over the same donor organ. Major concerns with LDLT are of donor safety and biliary complications. In conclusion, establishing a high volume LDLT centre with excellent success rates is feasible in the Indian setting.

ABSTRACT

In the last few years, there have been developments In many aspects of liver transplantation. Improvements in surgical techniques and immunosuppression markedly increased the success rates of liver transplantation. This success has lead to increasing numbers of recipients. However, the availability of cadaveric organs for transplantation has not been changed in the last 10 years, resulting in a growing discrepancy between donors and recipients. Thus, it is necessary to properly select the best candidates for a successful liver transplant. This article will review the indications and contraindications for liver transplantation in the Model for End Stage Liver Disease (MELD) score era.


En los últimos años han existido avances importantes en el trasplante hepático. La evolución en la cirugía hepática y la aparición de mejores inmunosupresores han incrementado de manera importante el éxito en el trasplante hepático. Este éxito ha aumentado el número de receptores; sin embargo, al mismo tiempo el número de órganos ha permanecido estable en los últimos 10 años, teniendo como resultado un incremento en la disparidad entre donadores y receptores. Por lo tanto es necesario conocer quiénes son los mejores candidatos para un trasplante hepático. En este artículo se revisarán las indicaciones y contraindicaciones en el trasplante hepático en la era de la clasificación de MELD (por sus siglas en inglés Model for End Stage Liver Disease).

ABSTRACT

To date, cadaveric organ donation is illegal in Egypt. Therefore, Egypt recently introduced living donor liver transplantation [LDLT], aiming to save those who are suffering from end stage liver disease. Herein, we study the evolution of LDLT in Egypt. In Egypt, between August 2001 and February 2004, we approached all centers performing LDLT through personal communication and sent a questionnaire to each center asking for limited information regarding their LDLT experience. We identified and approached 7 LDLT centers, which collectively performed a total of 130 LDLT procedures, however, 3 major centers performed most of the cases [91%]. Overseas surgical teams, mainly from Japan, France, Korea, and Germany, either performed or supervised almost all procedures. Out of those 7 LDLT centers, 5 centers agreed to provide complete data on their patients including a total of 73 LDLT procedures. Out of those 73 recipients, 50 [68.5%] survived after a median follow-up period of 305 days [range 15-826 days]. They reported single donor mortality. Hepatitis C virus cirrhosis, whether alone or mixed with schistosomiasis, was the main indication for LDLT. Egypt recently introduced LDLT with reasonable outcomes; yet, it carries considerable risks to healthy donors, it lacks cadaveric back up, and is not feasible for all patients. We hope that the initial success in LDLT will not deter the efforts to legalize cadaveric organ donation in Egypt

ABSTRACT

Liver transplantation has dramatically changed the management of patients with fulminant hepatic failure and subacute hepatic failure. A wide range of survival rates (55% to 90%) reported from different centers performing liver transplantation for fulminant hepatic failure reflects variations in patient selection and the centers' experience. Results of transplantation appear to be improving, with survival rates approaching 80% at 1 year in some series. The improved results are attributed to vigorous perioperative management, using intracranial pressure monitoring and continuous arteriovenous hemofiltration when required, early listing of patients with organ-sharing network, and exclusion of patients who are unlikely to recover neurologically after successful transplantation. Use of piggyback hepatectomy to minimize hemodynamic alterations during surgery has also contributed to better results in critically ill patients. Patients with acetaminophen toxicity have a more favorable outcome than those with viral hepatitis or non-acetaminophen drug hepatotoxicity. Because of limited availability of cadaveric organs, emergency living-related liver transplantation, use of ABO incompatible and marginal livers need consideration. Auxiliary liver transplantation is desirable for those who have a chance of spontaneous recovery, thus obviating life-long immunosuppression after recovery of the native liver.

ABSTRACT

End stage renal disease is not an absolute contraindication for liver transplantation (LT) in patients with end stage liver disease. Actuarial patient and graft survival are comparable for children and adults who undergo LT alone and liver-kidney transplantation (LKT). The most common indications for LKT are the primary hyperoxaluria type I (PH1) and the liver and renal polycystic disease. We report a 12 years old boy with congenital hepatic fibrosis with severe portal hypertension, encephalopathy and polycystic kidney disease with end stage renal disease on dialysis that underwent LKT. During the second postoperative week, he had a biopsy-proven acute liver and renal rejection, that had a good response to corticosteroids. Thirty days after surgery, the liver biopsy was without rejection. No other complications were observed (Rev Méd Chile 2003; 131: 1309-12).

ABSTRACT

Liver transplantation is accepted therapy for acute or chronic liver failure. Survival after LT has improved significantly in developed countries and this has increased the awareness of this treatment modality in the developing world. Successful LT in both children and adults have now been reported from India. Chronic liver failure secondary to cholestatic liver disease in the most frequent indication for LT, with biliary with atresia as the single commonest cause. Innovative techniques such as reduced size, splint, and living donor liver transplantation are being applied more often to decrease long waiting times and reduce associated morbidity and mortality. Early postoperative complications include primary graft failure, venous thrombosis, rejection, biliary complications and infections. Late complication includes CMV or EBV infections, side effects of immunosuppression, post transplantation lymphoproliferative disease and late biliary strictures. Most children achieve good quality of life. There are still many lessons to learn and there are future challenges such as the ever increasing problems of donor scarcity and the search for potent but less toxic immunosuppressive agents.

BACKGROUND/AIMS: End-stage liver disease caused by viral hepatitis C has been increasing recently in Korea. In this study, we investigated the clinical progress, recurrence, and management of hepatitis C patients who underwent liver transplantation. METHODS: We retrospectively reviewed the clinical progress and management of 16 patients (2.7%) with hepatitis C among 587 liver transplant patients from August 1992 to August 2002. RESULTS: Eleven cases among 16 patients were males. The median age was 56 +/- 6 (42-62) years and the median follow-up period was 6 +/- 13 (1-41) months. Thirteen cases underwent living donor liver transplantation and three had cadaveric whole liver transplantation. Clinical recurrence occurred in nine cases (56.3%) and mean time of recurrence was 5.2 months. Histological recurrence cases were eight (50%). A positive result of HCV RNA PCR was found in 90.9%, and all cases of clinical and histological recurrence in groups in the same periods were PCR-positive. Among eight cases showing histological recurrence, five patients were managed by ribavirin monotherapy, two patients received interferon and ribavirin combination therapy, and one patient was not treated at all. The serum aminotransferase level was normalized in six cases (75%) of them. CONCLUSIONS: We observed that the HCV reinfection rate of a transplanted liver was high in this study, as in other reports in the literature. The prevention of HCV recurrence and the management of post-recurrent cirrhotic change are crucial for graft and patient survival. We think customized protocols are needed for every situation of recurrent hepatitis C.
